Three reasons: it expired (most common), you typed it with wrong capitalization (codes are case-sensitive — always copy-paste), or it's one-use-per-account and you already redeemed it.

Rewards are permanent once claimed, but codes themselves expire. Some last 24 hours, some last weeks. Grab them as soon as you see them.
